The Pointe of Grace Care & Prayer Team invites you to a Bethany of the Northwest seminar that is designed to help us all understand why having conversations about end-of-life is important for ourselves and our loved ones.

Our presenters, Rev. Caitlyn Melillo (Director of Spiritual Care at Bethany of the Northwest) and Dr. Steve Jacobson (Retired Physician from the Everett Clinic) will focus on the well-known Five Wishes Form.

The Five Wishes Form is a legally valid living will (advance directive) that allows you to include your personal, emotional, and spiritual needs, as well as your medical wishes. If you already have your own “living will” you may consider attending to learn if this may serve you and your loved ones. “We have used the Five Wishes forms in previous congregations and in our own, personal, end-of-life planning and found it very helpful,” Pastor Coe and Janet Hutchison. Participants will receive a copy of the Five Wishes document.
